Abstract: – As healthcare institutions strive for digital transformation, the migration of legacy health systems—such as on-premises electronic health records (EHR), lab information systems (LIS), and imaging archives—toward cloud infrastructure has become imperative. However, this shift is complicated by stringent security, interoperability, and compliance requirements. This paper presents a comprehensive review of architectural models and frameworks for secure cloud transitions in legacy healthcare environments. It evaluates lift-and-shift, re-platforming, and microservices re-architecture approaches, and emphasizes threat mitigation strategies such as zero-trust security, end-to-end encryption, hybrid-cloud models, and compliance enforcement (HIPAA, GDPR). We identify best practices and highlight case studies that illustrate successful migration while maintaining continuity of care and data integrity.
